International Travel for AFS Students

AFS students are only able to travel outside the US with members of their host family
(age 25+) or on a school-sponsored/youth group-sponsored trip.

The first time a student travels internationally, a host parent will need to complete the
steps below. SP students are sponsored program students, the procedure for these
students is different and listed below, after the Core students instructions.

Core Students

Please send the following information to our Travel Coordinator, Katy Mulvihill at AFS
USA. (kmulvihill@afsusa.org ):
Student’s Name (First and Last)
Dates of travel
Destination (City, Country)
With whom will the student travel? (Host parents, school band, Youth Group)

As soon as this information is available, Katy will contact the student’s sending country to obtain
a signed parental permission form from the natural parents.

Submit student’s original DS-2019 (Sample attached) to the AFS-USA NY office, at the
address below to be signed. The form must be submitted using trackable mail, and the
shipment should include a pre-paid, trackable return envelope. Do not include
student’s passport.

New York, NY 10005

  • Please note if the student is a Sponsored Programs student, further instructions are required
    for submitting the DS-2019
  • *The DS-2019 (Certificate of Eligibility) is a letter sized document that most students fold, and
keep with their passport or other important documents. The signature and original document
are required for a student to re-enter the US.

Research and acquire any needed visas/documents required by the destination country and
cruise line (if applicable). Please note that some countries such as Mexico and Canada, as well
as some cruise lines, require minors to travel with a notarized letter from the natural parents
approving travel with host parents. This is different than the Parental Permission form required
by AFS.

Confirm that the student has an (M) on their US visa indicating Multiple Entry
